Bath Assessment of Subjective Quality of Life in Dementia (BASQID)
Before registering as a BASQID User, please read the following Terms and Conditions
Users agree that they will not modify, abridge, condense, translate, adapt, recast or transform the BASQID instrument in any manner or form including but not limited to any minor or significant changes in the wording , organisation or administration procedures for the BASQID instrument without prior written agreement of the authors Dr Richard Trigg, Professor Suzanne Skevington or Professor Roy Jones
You agree that you will not reproduce copies of the BASQID instrument except for the limited purpose of genereating sufficient copies for use in investigations stated thereunder and shall in no event disribute them to third parties by sale, rental, lease, lending or any other means or use them for conducting studies other than those specified in the BASQID User Agreement form.
Provision of Data
All data, results and reports obtained or prepared in connection with the BASQID shall remain the Users propery. However the authors may request the user to share the data, results and reports obtained through the use of the BASQID. This will be used to continue to develop the BASQID in UK.
Users are requested to send to the author Dr Richard Trigg a copy of their raw BASQID data on diskette or electronic file within 6 months of the end of the study. Before dispatch, Users should remove participant's names from the database and substitute a unique Study Identity code for each participant, to maintain confidentiality.Data contributed by Users that is included in publications will be acknowleged in the usual way.
Access to the Questionnaires
Once your registration has been accepted, you will be given access to documents relating to the BASQID. You should download all the files for the BASQID. These documents include:
- A copy of the BASQID interviewer score sheet
- A file containing suggested templates for question and response cards
- A user manual containing instructions for administrators
